You need a health certificate when you apply for a residence permit that will be valid for more than one year. The certificate is valid for six months beginning from the date of issue (Regulations on Administration of the Entry and Exit of Foreigners, State Council Decree No. 637, Article 16). For a permit of one year or less, the Regulations do not impose this requirement.
Where the requirement sits in the application
Article 16 of the Regulations sets out what a residence permit application involves: you submit your passport or other international travel document, qualified photos and material relating to the purpose of the application; you go through the formalities in person with the exit and entry administration authority of the public security organ of the local people's government at or above the county level in your proposed place of residence; and you provide biometric identification information such as fingerprints.
For a residence permit for study, Article 16(2) adds certification documents such as a letter indicating the duration of study, provided by the admission institution. The health certificate appears as a separate sentence in the same article, tied to the length of the permit, not to the reason for staying.
Who has to issue it, and what it has to say
The National Immigration Administration's service guide on issuing, extending, replacing and reissuing foreigners' residence permits states the detail. The guide is published in Chinese; the English wording below is this site's translation of that text.
It says the certificate must be issued by the local health and quarantine department, or by a health or medical department at or above the county level, and must confirm that the applicant does not have a serious mental disorder, infectious tuberculosis, or another infectious disease that may severely jeopardize public health.
Those three are the same conditions the Exit and Entry Administration Law lists in Article 21(2), among the grounds on which a visa shall not be issued. The two articles use the same categories, but they operate at different stages: Article 21(2) is part of the Law's rules on visas, while Article 16 of the Regulations governs the residence permit application made inside China.
The same section of the service guide states the deadline for the permit itself: where the visa you hold notes that a residence permit is required after entry, you must apply to the exit and entry administration authority of the public security organ in your intended place of residence within 30 days of entry.
What the official texts do not say
None of the documents used here states:
- how much the examination or the certificate costs;
- which clinics or hospitals are accepted, beyond the two issuer categories above;
- what the examination consists of;
- whether an examination or certificate obtained outside China is accepted, or whether a certificate needs translation, authentication or legalisation.
That leaves the practical questions — where to go, what to bring, what the certificate has to look like — to the exit-entry authority handling your case. Ask that office before booking anything.
Because validity runs from the date of issue, a certificate obtained well before you apply may no longer be valid on the day you submit it.

Do I need a health certificate for a one-year residence permit?
The Regulations require the certificate when the permit applied for is valid for more than one year (Article 16). They do not require it for shorter validity, and they do not state any other trigger.
Does the certificate have to be issued in China?

What if the certificate expires before I apply?
The six-month period begins on the date of issue (Regulations, Article 16). The texts do not describe any procedure for extending or renewing a certificate, so the practical answer has to come from the exit-entry authority.
Does a certificate mean the permit will be granted?
The certificate is one of the materials required for a permit valid for more than one year. It certifies the absence of the three conditions named above; it is not a decision on the application.
